Request a Sacramental Record

Please use this request form in any of the following circumstances:

1. You are trying to locate a record of your baptism, first communion, confirmation, or marriage.

​

2. You need a new certificate made to show that you received a sacrament.

​

3. A correction needs to be made to our records (for example, if we sent you a baptismal certificate with an incorrect date of birth). Please use the empty "Comments" box to indicate the corrections that need to be made.
